Voluntary Exclusion

An individual wishing to be excluded from casino gaming activities at all casino gaming establishments regulated by the Louisiana Gaming Control Board must appear in person at a Louisiana State Police, Gaming Enforcement Division location, and complete and sign the proper forms in the presence of a Division Agent. Instructions and the "Request for Self-Exclusion from Casino Gaming" form are below. Please read the instructions carefully.
