An individual wishing to be excluded from casino gaming activities at all casino gaming establishments regulated by the Louisiana Gaming Control Board must appear in person at a Louisiana State Police, Gaming Enforcement Division, location, and complete and sign the proper forms in the presence of a Division Agent.
